Before you make you next visit to Pointe Family Dentistry, please makesure to complete the following checklist in order for us to make your visit as smooth as possible. All related forms and links to complete this list are located at the bottom of this page, under the Patient Information section.
- Fill Out Health History Form (for new patients)
- Bring any new or changed insurance information.
- Contact your insurance provider to find out what your dentalinsurance covers and what you have left for the year.
- If your dental insurance will not cover a procedure you need or want performed, we offer dental plans at affordable rates.
- Apply for Care Credit if you think you will need financial assistance other than our dental plans.
- Take any pre-medications you may need.
- Brush, Floss, and Rinse with mouthwash!
- Arrive 15 minutes early to your appointment.
